Carbon falling into the chamber

Anyone ever have this happen? I’ve had it happen a few times. I think carbon falls down from the suppressor and gets in the chamber. Then it’s hard to chamber a round. Anyway to avoid? Or just need to clean suppressor more often?
 
It has happened to me. Dead Air Nomad Ti on a Tikka 6.5 CM T3x. Cleaned the can and no issues since .
 
Believe this happened to me on a deer hunt this year. Couldn't chamber a round. Luckily found a stick that was the appropriate shape to shove in the chamber and dislodge the carbon. I am not sure what the remedy is - clean the can more maybe?

I give mine the smack test.

If I can take the suppressor off and smack carbon chunks out of it by hitting it or tapping it on a table, it’s time.
