Serious question for the ones that have had failures.
Handloads or factory ammo?
For those who may wonder why that is important, the reason that most suppressors have barrel restrictions is due to the amount of pressure that needs to be contained by the blast chamber when the bullet "uncorks" from the barrel. Those barrel restrictions are predicated on using ammo that is compliant with SAAMI specifications. If you are one of the folks who loads their 300WM so that you can get 3000 fps with a 180 grain bullet out of a 20" barrel and believe it is OK because you don't have any pressure signs on your brass, you need to understand that you are operating outside of the design specifications of the suppressor that has a barrel restriction of 20" on a 300WM (not saying that was the case with the OP, just using it as an example).
