TRT

When guys say "full blood panels".....what does that mean to you?

Looking at options, there's a bunch of panels.

There are a ton of panels that can be run, so it would be best to ask the doctor you’re working with about specifics of each panel and which one would be best suited for your needs. This will also be a good indicator of the knowledge base and belief system of healthcare style that your doctor believes in.

Regarding the bloodwork specifically that I do yearly, my panel consists of TSH+T4F+T3Free (thyroid), CBC With Differential/Platelet (numbers/types of blood cells), Comprehensive Metabolic Panel, LP without VLDL+Chol/HDL+LD (cholesterol panel), Hgb A1c with eAG Estimation, Testosterone - Free and Total, Insulin, and Ferritin.

Hope this helps.

Testosterone (Free, Total) SHGB, Estrogen, Cholesterol, Liver/Kidney Enzymes, Red Blood cell counts, White blood cell counts for starters and in layman’s terms.

There’s plenty more they test for with “full panels”, hormones can be a complex game and if your Doc/Clinic is ONLY testing for testosterone… RUN. If your PCP is wanting you to inject anything less than 1x week to 3x week… RUN.

I have heard guys being on a 14 day and 30 day injection cycles. That’s a hormonal roller coaster that’s completely unnecessary.

I seriously recommend at a minimum 2x blood panels/year, my Doc has me on 4x panels/year to make sure we’re on top of things.

Been on TRT 3 years this August, I won’t stop. 1/2cc (100mg Test Cyp) pinned every 3rd day.
Fired my first Clinic, New PCP is on it.

For those of you who are on trt, what were your numbers when you started and where are they now?

Just had a blood panel done as im exploring this option. Everything came back in range except my cholesterol which was high and its always been good and my test level was 376.

Im almost 42. Looks like its on the very low edge of "okay"

I was at 307 when I started 4 years ago. It varies pending how much work I put in but ranges from 700-950 total.

I am interested about doctors prescribing TRT. This is the conspiracy theorist in me. Does your doctor recommend/ask about natural ways to boost T levels? What I am really asking is do they generally treat the whole person or a symptom?
I am leery to go to the doctors office because I disagree with some of their "health recommendations" when they are so often prescription based. I go for a wellness check once per year because a program at work pays me $50 to do it. Otherwise I try to stay away.
Personally I think there are far more prescriptions (not just to treat low T) than their needs to be when healthier choices can be made.
I just turned 41 and generally feel great. I had pneumonia last month and it wiped me out. I can only imagine thats what low T feels like, it was terrible. Not something I am interested in doing again.

Sounds like I need to do what Zap does. His blood work is awesome.

Please dont misinterpret this to suggest that TRT is never needed and nobody should use it.

I've chimed in a handful of times over the years and probably this thread.

Think I am in my 8th year of HRT. I've done hormonal self "boosting", done injectable. And have now advanced to cream. Some of my findings:

  • I am VERY sensitive to testosterone. Per my MD I am on one of the LOWEST doses out of all his clients. I simply process it differently for whatever reason.
  • At 45 my initial scan was maybe 405 total. I started post divorce and being in a mentally rough spot. Even out my moods and emotions. F'ing with hormones to assist in natural T production was bad juju. Anastrolzole and Clomophine was not good for me.
  • At 48 I went injectable. Got over my needle phobia. Never was a fan. I worked it out after a bit. A non issue now. Think Cypionate. The "usual"? The "Charts" generally "top out" at 1200 I think. I've been all over the place over the years. 1500+ once. Usually try to cruise 700-800.
  • At 53 my labs last month had me at 787 and 30.2 FREE. The Free is the "real" number and 30 is quite a bit (I am not a Pro. I have not followed along with all things Mens Hormones over the years. Don't take my word as gospel)
  • MY doctor is Top Notch. My former TRT salesman was a Gym Bro and sold you anything. Had waaaay too many clients to fully monitor well.
  • Labs- my new Dr- EXTREMELY thorough. Labs are EXTENSIVE. Thyroid, every lipid panel known, Vitamin D- He is VERY high on Vitamin D recommending ABOVE what most Dr do. I think many people are low. You simply can't soak up enough naturally. DHEA- runs that panel also. Another overlooked supplement.
My overall thoughts.....I suffered a Vertebral Artery Dissection Nov '23. Lucky to have not died. Stroke. Kinda forgot about it after a while. I most likely had a TIA last month. Right side got heavy....mini stroke. Stats say 1 in 5 suffer a major stroke follow up within 90 days. A shitty way to live mentally. I simply dont think I am going to last very long. Just my cards.

Blood pressure is up. Like not healthy up. Was running 140/100.

We didn't do a warm vacation this winter- which usually gives me motivation to lose a few pounds, eat better and clean things up. I did not. My job is very stressful and I am "on call" 24/7. That does not help. I occasionally would smoke 2 cigarettes on Friday after work to decompress (sorry...but soooo good) and have a handful of beers. My cholesterol is TERRIBLE. If HDL max is 1000...I think mine in last labs was 2980.

That mini stroke was a wake up call. I simply cannot live like that.

And when the medical providers tell you EVERY time "Eat better and get some exercise"- finding out at 50+ that is the biggest Bang for the Buck there is.

In less than a month of super clean eating and adding some long, slow cardio in...my BP is back down to 128/85ish.

In summary- would I do TRT again? Not so sure. My current provider is VERY good. Cutting edge. Reads lots of international studies. Very Huberman like. But he is expensive. He's $80 per month just as a retainer. His initial consult is a cash system and pretty spendy. But it will be a thorough assessment of your blood panel. You insulin resistance, your thyroid, your lipids, all your hormone levels and your Test. Most get so focused on Test levels they do not look at Estrogen. There is a balance ratio needed between them. And a T level of 1000 means shit if your Estrogen is way off.

My wife sees him as a client also. Been a game changer for menopause. She gets both Test and Estrogen.....and the D ! ;)

And there is a ton of shit that goes into mood, wellness and aging.

This is great info and reminds me of another question I had.
When TRT is prescribed is that usually a "life sentence"?
I assume it depends and most doctors have you do labs frequently to reassess the original plan.

Mine was 61.

Just because it is "in range" does not mean you wouldn't benefit from TRT. A progressive, knowledgeable doctor is who you need to be speaking with.

My numbers are now 900ish, I'm taking 400 mg weekly, twice the normal amount. At the regular dose, I would hover between 400-500. My doctor looks at it based on how I feel and if I have any adverse effects. Which I do not. So I'll continue at this level until otherwise directed.

At what point does it go from "TRT" to you're just on steroids?

I know plenty of people that got "TRT" from Dr's just to juice, in fact I know a Dr. (Whom, in an unbelievable stroke of irony, is named Chad) who specializes in this sort of thing.

Steroids are not a bad thing if they are managed by a Doctor. Looking at people like Rogan and Cam, sign me up.

I'm 51. My initial vel was 272 and the prescribed 1mg/week. My next blood work it was 630. Make sure they monitor your Estradiol. That can cause worse symptoms than the low T.

What about the supposed link between TRT and increased risk of stroke and blood clots?

My dad (76 years old) has been on injectable TRT for 25 years. Just recently had blood clots in his leg and lungs.

The TRT was the only thing the doctors could come up with that might have caused the clots.

I’m not convinced of that since he had taken it for so long, but here are a couple of links.

Most of the literature I saw was 10-15 years old so take that for what it’s worth.

Most of the articles and links saying there isn’t a correlation are from men’s health sources that are promoting and selling TRT and other treatments, so take that for what it’s worth.

I'm not sure at what point that is. There definitely are benefits for muscle building with higher amounts of test that are eye-raising. I guess if my test results were off the charts then I would be in the category. But in still within normal and have no side effects. But I'm slightly different than most guys here, since my health history involves a lot of cancer and radiation that caused the problem in the first place. Which is why my test results were so low to start with.

400 would be on the low side for guys who are really into bodybuilding, and I do compete in competitions. Some of the guys tell me all of the stuff they take and I just shake my head. They are literally ruining several of their internal organs and dont seem to really care.

There are a lot of steroids that are very safe to use, when used in amounts a doctor would recommend. But most guys get them on the black market and think "more is better". They then take more for longer amounts of time and thus, problems.

I've read quite a bit on it and I can't say one way or another if TRT is the culprit for the clots. There are several contradicting studies, so it's difficult go through all of them and figure out which study is right and which is wrong.

I've had people tell me all sorts of things about what they have heard regarding TRT. Generally, they are very uninformed.
