So...thickness is how manufacturers are getting higher R values. If you want more R you need more thickness. You could double up with another pad, maybe put a foam pad (Thermarest Z Sol for example) under the one you like. Or mess with the firmness of a thicker inflatable pad. Or try another all together, like an Exped for example (known for having high R values and very comfy).
 
I can't recommend an ultralight cot highly enough.

I do think that you probably need to assess the what and why of your discomfort though. It's hard for me to imagine being less comfortable on a thicker pad, especially in the low back.

The highest R-value and thinnest pad option is probably just a closed cell foam, but I feel like that would suck to sleep on as your only cushion. I will often double up an air pad with a closed cell foam, even on my cot. If not doubling pads, I will use a self-inflating air pad or just the foam on my cot. That might be the ticket for thin, R-value, and comfort for you.

While it's not what you're asking for, I have used the insulated Nemo Tensor for a few years and wouldn't hesitate to recommend it also.
 
I sleep well on Klymit static V design. I have an insulated model from circa 2018. Come to find out it has an actual R value of less than 2.

I’ve tried various different 2.5-3” thick air pads include therma rest. Felt like I was on a small version of those cheap blow up mattress despite changing the internal air pressure. Lower back and neck don’t agree with that style

Klymit has been known to greatly inflate their r values in the past so I’m trying to stay away. Any other thin pad, decent r value suggestions?

Klymit makes an insulated Static V.....R value of around 5 if I remember right. I've slept to 0 degrees on mine.
