My vote is for the Swarovski Z5 3.5-18x44 with the Ballistic Turret. Fits all the criteria you list. I own 5 of them on various rifles and have used them from Mexico to the Arctic Circle. Those that list them as unreliable are full of beans and have probably personally never used one in the hunting fields.
