For some who want to add lots of weight up front, will do this also, I think mine worked out to about 20 grains. Part of Ashby's way of beefing up the front end. Henro on AT has some good write ups on doing it.

Watched a youtube Video on it. Cut it, taper it, glue it on. Finding correct fit looks like hardest part

As an "outsert" design, ya it could be one piece, but with HIT's you couldn't connect the two together. The original outserts that came with the Trohpy Ridge arrows did this a little bit but only covered about 1/4" of the outside of the end of the shaft. But the outsert itself wasn't very strong and would snap inside the shaft on hard hits. That's why I changed them all over to HIT's which work a heck of a lot better than outserts IMO. No breaking or bending with the HIT's.

Is that an actual footing that sleeves the arrow end or just an outsert that fits the smaller diameter shaft?

It sleeves the outer end of the arrow. I really like the idea so far and I believe it strengthens one's setup quite well. I shot a caribou with the arrow last week, with a mech head and it zipped through him like a razor sharp fixed blade. I was pretty impressed.
