Slug gun issues

Hey guys I have been hunting with the same 12 gauge Benelli slug gun for almost 20 years and have had the same Leupold scope fail now for the 3rd time. I don’t like the fact that the only base and rings have been weaver style especially with the recoil of a 12 gauge. I have bought a base and ring all in one and had the bottom milled to match the weaver base and attached another scope and it will shoot a 1 inch group and then track off to one side then another 1 inch group and the track off vertically. I am hoping that someone can help me figure out what is going on.

Are you using loctite ? Have you sent the scope back " Leupold has a lifetime guarantee, if it's an issue with the scope they will fix or replace it.

Is the scope base attached to the receiver or barrel?
