I am guessing that because it is a revolver most of the gunpowder was not igniting and what was igniting was escaping through the rear.

Also could have been reloads with no gunpowder only primer...I have been in a hurry while reloading and done that before...the projectile gets stuck in the barrel and if you don't notice, it would be easy to do it multiple times....would have to be REALLY dumb to do it 8 times though.

I've seen 13 stuck in a barrel....8 times that is only 1 reload. Imagine reloading 2x
