...you don't, it doesn't exist. Buy a TightSpot, and then depending on your spine & O.D. you can rotate the pressure wedges 90° to increase tension even more if needed.

I own a tight spot, much prefer the two piece.

I used to wrap a rubber band around the bracket and gripper. This pushed the small diameter arrow out towards the opening and the smaller pinch point of the opening.
